Scheduling Your Visit

To schedule an appointment, our Foothills Medical Group physicians and providers each have a direct phone line for easy access to their individual offices or logon to our patient portal.

If it is necessary to cancel an appointment, please notify your Foothills Medical Group provider’s office as soon as possible or within 24 hours of the scheduled appointment. This courtesy will provide us with an opportunity to care for another patient in your place.

Preparing For Your Visit

On your initial visit to Foothills Medical Group, please arrive at least 30 minutes prior to your scheduled appointment so that you have appropriate time to register with our receptionists. You will be asked to complete a registration form that will allow us to establish your medical record. You will also be asked to provide staff with your current health insurance information. We ask that you please bring your current health insurance card(s) and a list of all of your prescriptions and medications to this visit and all subsequent visits, as well as your co-payment, if applicable.

On future visits, please check in at the reception desk so that Foothills Medical Group staff can assure the continued accuracy of various personal and health insurance information. We appreciate your notifying the receptionist or doctor's office of any changes in names, addresses, telephone numbers or health insurance coverage since the time of your last visit. Verifying this information at each visit will ensure that you receive the most appropriate medical care, and will also ensure that the services you received are covered by your health insurance company.

Again, please bring the following with you to your Foothills Medical Group visit:

  • A list of your current medications
  • Any and all x-rays that pertain to your visit with us
  • Health Insurance information and card(s)
  • Information about what treatments and medications have been administered to you in the past
  • Medical records from prior visits that you think may be helpful to your Foothills Medical Group healthcare provider

Appointments and Scheduling Office Policies

How do I make an appointment?

To schedule an appointment, please call one of our Foothills Medical Group offices or logon to the patient portal. Please have the following ready:

  • Patient's name
  • Patient's date of birth
  • Reason for appointment
  • Patient's day time and evening phone number
  • Patient's address
  • Patient's health insurance information
  • Patient's email

When should I call the doctor?

Primary Care physicians should be the first doctors you access when you feel sick or ill. You should call whenever you feel you have a significant health problem that you believe requires a physician visit, or about which you are unsure or uneasy.

When calling, what information should I provide?

When you are sick and call a Foothills Medical Group office for an appointment, we need to know exactly what your symptoms you are experiencing. This allows us to best understand your healthcare needs. Foothills Medical Group staff has been trained to ask questions to assure that you can be seen quickly and appropriately.

What paper work can I complete before I arrive at the office?

Before the doctor can see you, certain paperwork needs to be completed.  Because the completion of these forms and questionnaires take time to complete, they have been posted on our Foothills Medical Group website for your convenience and can be printed.  To save you time, we suggest that you complete the following forms before coming to a Foothills Medical Group office. Click here for patient forms.

Can I just walk in for a doctor's visit?

At our Mt. Jewett Health Center in Mt. Jewett, Pennsylvania, walk-in patients are accommodated. For other Foothills Medical Group practices, same day appointments are accommodated but patients must call in advance.

What if I have to change/cancel my appointment?

We appreciate you notifying Foothills Medical Group if you cannot make your scheduled appointment so that we have the opportunity to schedule other patients during that time.

If you need to cancel an appointment, please call 24 hours in advance. We appreciate your understanding of the fact that Foothills Medical Group reserves the right to terminate our relationship with patients as a result of repeated missed appointments and late cancellations.
